Job Title: Home Health Registered Nurse RN Full TimeJob Summary:We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Registered Nurse, you will provide high-quality nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.Responsibilities:

• Develop and implement individualized care plans for patients, incorporating medical interventions and measurable goals and outcomes.
• Provide education and support to patients and their families, promoting independence and self-care.
• Collaborate with primary caregivers to ensure optimal functioning and care for patients.
• Coordinate continuum of care through timely communication with patients' physicians and other caregivers.
• Provide skilled nursing care in patients' homes, utilizing a range of skills including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.Requirements:
  • Active RN license in the state of application.
  • Valid CPR certification.Preferred Qualifications:
    • Medicare Skilled Nursing experience.
    • Basic understanding of OASIS.
    • 1-year RN experience in a healthcare setting.
