Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and barriers to care, developing personalized
care plans
Coordinate care between members, families, and providers, ensuring
timely access to services
Monitor member progress and revise care plans as necessary to meet
goals
Required Qualifications, Training, and Education
Degree from an Accredited School of Nursing or Bachelor's degree in
Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse State Licensure in Oregon and Washington
Availability to work from 8 am to 9 pm PST
Experience with social determinants of health and community
resource navigation preferred
